LPS of USA Executive Board Nomination Form
(Term 2025 – 2026)

Instructions:
(1) This Form shall be used for self-nomination only and must be submitted before deadline.
(2) A board of director can self-nominate himself/herself for maximum of 2 positions on the Executive Committee, however, only 1 position for an officer.
(3) The candidate must be physically present during voting process.
