LPS of USA Board Nomination Form
(Term 2025 – 2026)

Instructions:
(1) The Purpose of this election is to elect a Representative from the LPS Samaj/Area to represent the LPS members in the State/Region on the LPS of USA board for a one-year term
(2) Any Representative that would like to serve on the LPS of USA Board must hold a Lifetime Membership in the organization and be a minimum 18 years of age.
(3) Annual Members are not eligible to serve as an LPS of USA Board member.
(4) A member must be present to vote and shall not be allowed to vote by proxy.
(5) The election results with name(s) and all other required details below shall be submitted before deadline.
